GCFC Soccer Registration

This site will allow you to register a player and submit an online payment. Please fill in all of the required information and ensure that all information is entered accurately. To complete the registration, you will be required to submit a payment on our secure site. Please have your MasterCard or Visa credit card information available. Once the online registration payment has been processed, you will see a charge on your card from German Canadian Football Club. A receipt will be emailed to you using the email address you provided.

WAIVER AND RELEASE OF CLAIMS

Please read this carefully. I am aware that in registering myself or my child for participation in German Canadian FC event(s), such as, but not limited to, clinics, tryouts, practices, games, tournaments, and events, I will be waiving and releasing all claims for injuries my child might sustain arising out of the event(s). I recognize and acknowledge that there are certain risks of physical injury to participants in the event(s) and I agree to assume the full risk of any injuries, including death, damages or loss, regardless of severity, which I or my child may sustain as a result of participating in any and all activities connected with or associated with such event(s). I agree to waive and relinquish all claims I or my child may have against the German Canadian Football Club/ German Canadian Club of London and coaches, managers, officers, directors, agents, servants, volunteers, affiliates, and employees as a result of participating in the events(s). I do hereby fully release and discharge the German Canadian Football Club and its officers, directors, agents, servants, volunteers, affiliates, and employees from any and all claims from injuries, including death, damage or loss, which I or my child may have or which may accrue to me or my minor child and arising out of, connected with, or in any way associated with the activities of the event(s). I further agree to indemnify and hold harmless and defend the German Canadian Football Club and its officers, directors, agents, servants, volunteers, affiliates, and employees from any and all claims resulting from injuries, including death, damages and losses, sustained by me or my child, arising out of connected with, or in any way associated with the activities of the event(s). We hereby authorize the German Canadian Football Club to use my name or image or my child’s name and images, pictures and/or reproductions of my child while engaged in games or related activities in the club’s promotional material and/or on the website. I have read and fully understand the waiver and release of all claims.

The Participant acknowledges that this Agreement may be accepted in electronic form and that it has the same force and effect as if executed in non-electronic form. The person executing this Agreement represents and warrants that they have legal capacity to enter into this Agreement and be bound by its terms.

IMAGE CONSENT, RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS AND INDEMNITY AGREEMENT

WARNING! THIS IS A BINDING LEGAL AGREEMENT. CLARIFY ANY QUESTIONS OR CONCERNS BEFORE SIGNING. BY SIGNING THIS DOCUMENT, YOU WILL WAIVE CERTAIN LEGAL RIGHTS. PLEASE READ CAREFULLY!

This Agreement must be executed by the Participant and/or the Participant’s parent/guardian (if applicable, when the Participant is younger than 18 years old) prior to participation. The Participant acknowledges and agrees to the terms outlined in this document. When applicable, the Participant’s parent/guardian acknowledges and agrees to the terms on behalf of the Participant and references in this document to the Participant agreeing to or acknowledging a risk or term is understood to be referring to the Participant’s parent(s)/guardian(s) agreeing to or acknowledging the risk or term on behalf of the Participant.

ACKNOWLEDGMENT

The Participant acknowledges and agrees that they have been provided with sufficient opportunity to review and consider the terms and conditions of this Agreement before signing. After this review period, the Participant voluntarily accepts the terms and conditions without undue influence or duress. The Participant affirms that they are fully aware of the risks and hazards associated with the Activities offered by the Released Parties, including but not limited to physical injury, illness, and, in extreme cases, death. The Participant acknowledges that participation in any Activity conducted by the Released Parties is entirely voluntary and that they choose to participate despite the risks. The Participant agrees to attend, if requested by the Participant, a pre-activity briefing for risk and terms explanation.

THIS DOCUMENT IS SIGNED VOLUNTARILY AND WITHOUT DURESS, FRAUD, UNDUE INFLUENCE, OR IMPROPER INDUCEMENT.

The Participant acknowledges that they have read and understand this Agreement, that they have executed this Agreement voluntarily, and that this Agreement is to be binding upon themselves, their heirs, spouse, children, parents, guardians, next of kin, executors, administrators and legal or personal representatives. They further acknowledge that by signing this Agreement, they have waived their right to maintain a lawsuit against the Released Parties on the basis of any claims from which they have released herein.

I authorize Canada Soccer, Ontario Soccer, the applicable District Association and Soccer Organization to collect and use personal information about me for the purpose of receiving communications from the Canada Soccer, Ontario Soccer, District Association, and Soccer Organization. I understand that I may withdraw such consent related to receiving communications at any time by contacting the Ontario Soccer Privacy Officer at privacy@ontariosoccer.net or by mail to: Attention: Privacy Officer, Ontario Soccer, 7601 Martin Grove Road, Vaughan ON L4L 9E4. The Privacy Officer will advise the implications of such withdrawal.

REFUND POLICY

Please read this carefully. The German Canadian Football Club does not provide full refunds after payment has been made. Refund requests must be made to the GCFC executive board for review and approval. Refund amounts will be determined by the GCFC executive board. At minimum, all GCFC fees incurred for player registration will be held back. If a player has participated in any games, no refund may be available.
